Welcome Home Package

This is a way for your senior loved one to easily transition home after a hospital stay, rehabilitation program, or unexpected time away. They may be thrilled to return home, but it can be overwhelming and even frightening. Normal, everyday tasks could become difficult.

Our in-home caregivers are here to help. As part of the Welcome Home Package, your senior loved one will enjoy care services such as medication pick-up/reminders, transportation to doctor’s appointments, and three weeks of regular check-ups, housekeeping, and meal preparation.

Hospital To Home Transitioning Care

Our senior care services also include helping seniors in the Los Gatos area transition back to their home after hospital discharge. A hospital discharge can often mean more responsibilities for you. To ease some of the stress, we offer our in-home care service to help them successfully make the transition. This will prevent your senior loved ones from doing poorly during the transition and needing to return to the hospital as a result.
